The rail/wire runs through the hole on the loop cap in one continuous line to a terminal post. If using barb wire, you should not use a loop cap; use a line post barb arm instead, which also has a loop for the top rail. Aluminum loop caps are typically used on residential chain link fences, while steel caps are used on commercial/industrial projects.
